Unique identifiers are only collected to verify a user\u2019s identity. Personal information collected consists of full name, degree, contact address, contact phone number, specialty, last four digits of the social security number and email address. Email addresses are used for the sole purpose of communication regarding Eastern Carolina ENT and for purposes of notifying users of ENT updates and future programs offered through Eastern Carolina ENT.<\/p>\n\n\n\n<p>Cancellations must be made in writing and received by the Eastern Carolina ENT Office no less than 1 (24 hr) business day prior to your appointment. If this Cancellation is received in the time-frame indicated, a Refund will be made to the registrant as outlined in the program brochure.
